Get a referral

To ensure we are able to reach those who need us most, patients can self-refer or be referred to us by a Healthcare Professional.

How to get referred

If you think you or someone you know could benefit from our services, please talk to your Doctor, Community Nurse or Social Worker. There is a rapid and uncomplicated referral process for medical professionals to follow. Don’t forget, we’re a charity and all our services are free.

If you have been diagnosed with a life-limiting illness, you can also refer yourself to our services by completing a referral form here, or calling 01753 848925.

Need to chat?

If you would like to talk to us first about our services, please call our Single Point of Access Team on 01753 848925.

If you would like to have a look around our facilities, please get in touch and we would be happy to give you a tour. Or take a virtual tour of our Hospice and facilities without leaving the comfort of your home.
