An introduction to Advance Care Planning (ACP)

Session length: 3 hours, 1.30pm – 4.30pm

An exploration of the wider concept of Advance Care Planning (ACP). This afternoon session provides information about the different aspects of advance care planning, including ReSPECT forms. You will have the opportunity to think about your role in ACP, helping you to think about how we share this important information and the difference it can make to patients and families.

This session is open to clinical and care home staff who wish to learn more about ACP. 

This session will be delivered at Thames Hospice. Light refreshments will be provided.
