We offer assessment, complex symptom control, emotional support, advanced care planning and short term personal care to people over the age of 16, diagnosed with a life-limiting condition and registered with a GP surgery in East Berkshire and South Buckinghamshire.
The team comprises doctors, nurses and paramedics with specialist skills. We work closely with other members of the wider multi-disciplinary hospital and community teams to ensure seamless care and good communication between all those involved in looking after our patients.
All referrals received from our community partners are assessed in one of our dedicated assessment clinics at Thames Hospice. If patients are too unwell to attend a clinic appointment, one of our Clinical Specialists will visit at home to complete their assessment.

We also signpost to services such as occupational therapy, complementary therapy, medical supplies and for financial support (when applicable) to enable patients to live comfortably and safely in their own home, avoiding hospital admissions wherever possible.
